The ENSMHA Covid-19 Acknowledgment & Liability Release Waiver must be completed online, prior to entering the arena for every skill session, practice or game. Failure to complete the waiver will result in no admittance to the arena. A Covid Ambassador will be stationed at the arena entry point and will have a real time digital version of who has completed the form.

THIS IS MANDATORY. A NEW FORM MUST BE COMPLETED EACH DAY THAT ENTRY TO THE ARENA IS REQUIRED FOR EACH PARTICIPANT.
