Vaccine Verification per OMHA Policy
Active screening will begin at both arena locations this week in order to meet the mandated vaccination status set forth by OMHA, to be enacted on October 31st, for those eligible players and bench staff.  Please be prepared to show proof of your vaccination status upon entry to the building.  This screening is a one-time occurrence that will be monitored by a team representative and a member of the ENSMHA Board of Directors.  Proof of vaccination is a requirement to roster players to a team.

The following process must be completed by all players and bench staff prior to the October 31st deadline, however we are hoping to have completed this process by October 25th.

1. Sometime between today and October 31st when you are attending an on-ice session for your team you will be asked to enter your required information on the Ontario Government website and then show proof of your vaccine status to the team representative and board member present.  Your team representative will let you know what date this will occur.  You will need a valid health card to retrieve this information, or you may use the new QR code from the Government of Ontario website to be scanned for approval.

 

2.  If you are fully vaccinated (2 doses + 14 days), your name will be checked off on a team list - we will not be saving any of your personal information, nor a copy of the vaccine status as shown.  We simply need to verify that you qualify to be rostered.

 

3. The association will be providing iPads at the check in to facilitate this process, and they will be sanitized after each use.  You will be responsible for entering the information - no team representative or board member will be permitted to touch your health card or retrieve the information on your behalf in order to maintain the highest level of privacy possible.  

 

IMPORTANT: Your existing copy (whether paper or electronic) of your vaccine status (pdf) that you may be currently using to enter the facilities WILL NOT suffice.  Each player/bench staff must enter their information individually at the time of the screening.

 

4. For any player/bench staff who do not/will not meet the requirements, please be advised that you will be suspended from on-ice activities as of November 1st and cannot be rostered on a team.  It will be at the coach's discretion to discharge you from the roster should you not attempt to meet the guidelines within a reasonable time frame, as approved by the Board.

 

Please expect delays in entering the arenas while we work through this process.  We ask that you give the team reps and board members the utmost respect.  They are volunteers and this is a necessary step that we must follow.
